Practical, personal support to help you stay independent, comfortable and connected in your own home and community.
Support at Home is the Australian Government’s in-home aged care program for eligible older Australians who need support to continue living safely and independently at home.
The program started on 1 November 2025, replacing the former Home Care Packages program. It is designed to make in-home aged care simpler, more consistent and better matched to each person’s individual needs.
The support you receive is based on your assessed needs, approved care plan and available funding. Depending on your circumstances, this may include personal care, help around the home, transport, social support, nursing and other approved services.
Support at Home organises services into three broad areas: clinical care, independence support and everyday living support. This helps make it clearer what types of support are available and how they fit into your care plan.
The aim is to give older Australians more choice, clearer support and the right help to stay connected to their home and community for longer.
We support older people who want to remain safe, independent and connected at home and in the community. Support is tailored around each person’s needs, preferences and approved care arrangements.
Start by contacting My Aged Care to check your eligibility and begin the assessment process.
An aged care assessor will review your needs and help determine the level of support that may be suitable for you.
If approved, you will receive information about your care arrangement, the types of support available to you and the next steps.
Once your funding or care arrangement is in place, you can contact Disha Community Care to discuss the services you need and get started.
We can help you understand the My Aged Care process, what information you may need, and what to expect when applying for government-funded aged care support.

Your care plan helps identify the services you can access, including personal care, home support, community access and other eligible supports.
Depending on the type of service you receive and your financial circumstances, you may be required to contribute towards the cost of some supports.
Support at Home funding is provided in quarterly budgets. If you have money left at the end of a quarter, you can carry over up to $1,000 or 10% of your quarterly budget, whichever is higher, into the next quarter. Different rules apply to unspent funds carried over from the former Home Care Packages program.
Your Support at Home classification can be reviewed if your needs change. You may need a reassessment if you require a different level or type of support. You can request a reassessment through your aged care provider or My Aged Care.
